Some quick hits !

1) Urinalysis with Reflex to Culture orderable – AT LAST
Correlation isn’t causation but I will say that Dr. Creedon’s incessant consternation about why this order didn’t exist to everyone who would listen might have willed this into existence.
Now, when ordered, a positive UA will automatically reflex to culture. What is a positive UA? Glad you asked!
This is the labs breakdown!
Now we need to think about placing this order this carefully. Remember in some instances we might want a culture regardless of UA (think neonatal fever, neutropenic pt with sx). Other times we might not want a culture at all.
So:
A) It has been added to the quick list of labs (see above pic)
B) It has been added to appropriate panels – Acute Scrotal Pain, Ectopic Pregnancy, STI Testing Panel
C) It has been added to appropriate ordersets – Abdominal Pain, Gastroenteritis
D) It has not been added to Septic Shock, Neonatal Fever, Fever & Neutropenia
But this is a whole new world so if you spot places it needs to be switcheroo’d, let us know!

 

2) ED Work-up Speed Button Smartphrases –
We talked about adding documentation speed buttons to the Work-up tab in a prior message.
We have now made some sample workup smartphrases available to all. Look for .EDWU___ in the departmental smartphrase

3) Longwood Pediatrics Patients Being Admitted –
Longwood Pediatrics is a private office that still has admitting privileges.  Unfortunately their patients often get admitted to PHM accidentally.
Moving forward if we place a Bed Request order to PHM on a Longwood Pediatrics patient, this warning will come up prompting a discussion with their on-call physician to discuss if they want to be the attending on the floor.
If after discussion, they do want to attend, the Service on the Bed Request Order needs to be changed from PHM to General Medical – Private.
The COPP will still place on a PHM care team and we will still sign out to a PHM resident team.
